Click on the links below to access model FCRB summary forms used by several Tennessee courts and created in compliance with Rule 403(d)(1) of the Tennessee Rules of Juvenile Practice and Procedure. These forms were developed with the assistance of model FCRB facilitators and were designed to address the required review criteria under Rules 402(b) and 403. These forms are available to any court, and the Court Improvement Program is available to assist courts with this and any other aspect of FCRB.
