Forms are in Adobe Acrobat ® (PDF) format ONLY. They were designed for version 4.0 or higher. If you are having difficulty opening or completing any of the forms, please obtain a more recent version of Acrobat Reader ®. If you need the free reader file it is available here in the Windows format . Other systems should go to the Adobe ® site, for their version.
Forms cannot be submitted electronically. They must be filled out and printed, then mailed or hand delivered to the proper authority.
You may complete the forms in any of three ways:
1. Print and Complete by hand or typewriter:
A form may
be printed from your browser, then typed or filled in by hand.
2. Complete While On-Line:
A form may be filled out
while online, then printed from your browser. In the event of a crash of your
browser or disconnection from ther internet, you will have to start the
procedure over.
No work can be saved.
3. Save and Complete on Your Computer:
If your browser
is set to open Acrobat Reader within the browser window, you choose
File, Save As from your browser's menu bar. Save the form to a
folder on your computer (Be sure to notice where it is saved!). Open Acrobat
Reader® on your system, then open and complete the form. You
will be able to print, but not save, any changes to the original blank form.
This form may be re-used but no entered information, such as names and
addresses, can be saved.
Forms cannot be sent electronically. Print, sign and date the form, make necessary copies and place in the mail or hand deliver.
