Claim Forms

You cannot submit these forms on-line. They must be printed on your printer and mailed or hand delivered to appropriate office as indicated on the form. If you wish, you may fill them out as presented online, or you may right click on the link and save the file to your system. These forms perform all totaling and other calculations automatically. In order to take advantage of this feature, you must enter all information and then print out your form to be sent to the AOC.


Adobe PDF Format

These forms are in the Adobe Acrobat® format (PDF). They are not word processor files and will not open in WordPerfect or MS Word. If you need the free reader, it is available for download from the Adobe Acrobat web site.

You cannot save a form and then return to update it later unless you have the full copy of Adobe Acrobat. You will not be able to save the file once you have entered information into it.

These forms are intended to collect all relevant information for a client in one session, and, once information is entered, they may not be saved unless you have the full copy of Adobe Acrobat.

If you need to enter information into the form on subsequent dates, you will have to print out the blank form and enter information by hand, foregoing the automatic calculation built into the online form.

Please notice! - Secure, by means of a staple, all pages of these forms, together with any required attachments before mailing them to the Administrative Office of the Courts.
Failure to do so will delay the processing of your claim!
